The Youth Transition Worker delivers one-to-one in-home and community setting service to youth 16 - 24 years of age who are exiting the child welfare system, a mental health centre or treatment program, and who require assistance identifying, accessing, and navigating adult service systems related to:
Using a wrap-around service model that is responsive to individual needs and goals, youth can build capacity and resilience as they transition from care to independent living.
All staff have a vulnerable sector screening and a combination of education and experience needed to support positive outcomes for children and youth.
